Appointments
We see patients by appointment and we also accept walk-ins for patients with urgent needs. Patients with appointments are seen first.
If you are unable to keep a scheduled appointment, we ask that you please give us 24 hours notice so that we may give your appointment to someone else in need.
Prescription Refills
For prescription refill, please contact us with the following information:
- Pharmacy Name & Phone #
- Name of Prescription & Dosage
Please allow 48 hours to process prescription refill requests. As a general policy, we do not request refills after hours or on weekends.
Patient Registration
If you are new to our practice, we'll need to gather some information from you. Click here to download the new patient registration form.
For your first appointment, please also bring a government issued photo ID and your insurance card if you have insurance. Please also remember to bring a list of any medications that you are currently taking, including any over the counter medications.

We accept most major health insurance plans. To confirm that we accept your insurance plan, we encourage you to contact our office

At the time of your visit, please be prepared to pay any co-payments, deductibles or non-covered services. For your convenience, we accept cash, checks and debit/credit cards.
